Advances in the Management of Bacterial Septic Arthritis

چکیده
The clinical presentation of a patient with one or more hot, swollen joints is common. The differential diagnosis is broad but the most serious potential cause is bacterial septic arthritis. The management of bacterial septic arthritis relies on early recognition, diagnosis and timely drainage of purulent material, together with prompt administration of antibiotic therapy. If the diagnosis is not made rapidly then the treatment of septic arthritis may be delayed, which can lead to substantial morbidity due to catastrophic joint damage [1], as well as significant mortality due to overwhelming septicemia [2]. The differential diagnosis of bacterial septic arthritis includes inflammatory arthritis, crystal arthropathy, trauma, hemarthrosis and degenerative joint disease. Even in the hands of experienced physicians, the crucial diagnosis of septic arthritis can be a difficult one to confirm. Despite advances in laboratory techniques, efforts are still being made to find a synovial or serum investigation of sufficient sensitivity and specificity to clinch the diagnosis. In addition, the emergence of unusual and resistant organisms makes the management of septic arthritis an ongoing challenge.

Despite advances in antimicrobial and surgical therapy, septic arthritis remains a rheumatologic emergency that can lead to rapid joint destruction and irreversible loss of function. In adults, Staphylococcus aureus is the most common microorganism isolated from native joints. Streptococcus gordonii is a prominent member of the viridans group of oral bacteria and is among the bacteria most freq...

Septic arthritis is a rheumatologic emergency as joint destruction occurs rapidly and can lead to significant morbidity and mortality. Accurate diagnosis can be particularly challenging in patients with underlying inflammatory joint disease. This review outlines the risk factors for septic arthritis and summarizes the causative bacterial organisms. We highlight advances in antibiotic management...

Background: Triggering receptors expressed on myeloid cells i.e soluble triggering expressed on myeloid cells-1 (STREM-1) is inducible on monocyte/macrophages and neutrophils and accelerates tissue destruction by propagating inflammatory responses in disease related to bacterial infections. The aim of this study was to investigate the role of STREM-1 in the synovial fluid to identify septic...
